Health Disclosure & Liability Waiver
Please read and confirm the following. This waiver protects both you and our therapists by making sure your treatment is safe and appropriate.
1. Health disclosure
By booking a session, I confirm I have disclosed any of the following that apply to me:
- Current pregnancy and trimester (if applicable)
- Recent surgery, injury, or physical trauma
- Skin conditions, open wounds, or infections
- Cardiovascular conditions (e.g. high blood pressure, heart conditions)
- Blood clotting disorders or a history of blood clots (e.g. DVT)
- Cancer (current or recent treatment)
- Diabetes
- Allergies (including to oils, lotions, or scrub products used in treatment)
- Any other medical condition that may affect the suitability of massage or spa treatment
I understand that not disclosing relevant health information may affect the safety and outcome of my treatment, and that Jemari Jelita Mobile Spa is not liable for complications arising from undisclosed conditions.
2. Prenatal & postnatal services
If booking a Prenatal Massage or Confinement / Postnatal Care package, I confirm that:
- I have consulted my doctor or healthcare provider and received clearance for massage therapy, where applicable.
- I will tell my therapist immediately about any discomfort, pain, or unusual symptoms during the session.
- I understand our therapists are not medical professionals and our services do not replace medical care or advice.
3. Assumption of risk
I understand that massage and spa treatments, like any physical therapy, carry some inherent risk (e.g. temporary soreness, allergic reaction to products used). I voluntarily choose to proceed and accept this risk.
4. Consent to treatment
I consent to receive the massage / spa treatment I have booked, performed by a therapist assigned by Jemari Jelita Mobile Spa, at the location I have provided.
5. Right to modify or decline treatment
I understand that Jemari Jelita Mobile Spa and its therapists may modify, pause, or decline treatment at any time if it is deemed unsafe based on my health disclosure or a condition observed during the session.
6. Limitation of liability
I agree that Jemari Jelita Mobile Spa, its owner, and its therapists shall not be held liable for any injury, complication, or adverse reaction resulting from:
- Undisclosed medical conditions
- Failure to follow pre- or post-treatment advice given by the therapist
- Circumstances outside the reasonable control of the therapist
7. Acknowledgement
By proceeding with booking and/or treatment, I confirm that I have read, understood, and agree to the above.
